Important Enrollment Deadlines

If you don’t enroll in the following plans before your 91st day of employment, you will not be able to enroll until the next Annual Enrollment period unless you have a qualified status change. If you enroll by your deadline, coverage begins on your 91st day.

 Full-time Hourly and Salaried 
Medical/Vision, Dental,  Spending Accounts and Critical Illness Protection Plan To have coverage this year, enroll before your 91st day of employment.
Disability Coverage* You are automatically enrolled in this plan. If you do not want coverage, you must opt out before your 91st day of employment.
Life Insurance You have a one-time opportunity to enroll in Basic and Supplemental Life Insurance (coverage of up to three times your pay not to exceed $500,000) without having to provide an Evidence of Insurability form if you enroll before your 91st day of employment.
All Other Plans For all other benefits, you can enroll throughout the year once eligible.

* Salaried associates are automatically enrolled in a Company-provided disability plan and will be defaulted into the Tax on Plan Cost option for LTD.
