New Associates

Important Enrollment Deadlines

If you do not enroll in the following plans before your 29th day of employment, you will not be able to enroll until the next Annual Enrollment period unless you have a qualified status change. If you enroll by your deadline, coverage begins on your 29th day.

 Full-time Hourly and Salaried Associates 
Medical, Dental, Vision, Medical Payment Plan, Spending Accounts and Critical Illness Protection
To have coverage this year, enroll before your 29th day of employment. You will automatically be enrolled in associate only Medical Coverage, under the Kaiser Permanente Point of Service option. If you do not want coverage you must complete the legally required HC-5 form and opt out of coverage during your enrollment.
Disability Coverage* You are automatically enrolled in this plan. If you do not want coverage, you must opt out before your 29th day of employment.
Life Insurance You have a one-time opportunity to enroll in Basic and Supplemental Life Insurance (coverage of up to three times your pay not to exceed $500,000) without having to provide an Evidence of Insurability form if you enroll before your 29th day of employment.
All Other Plans For all other benefits, you can enroll throughout the year once eligible.

* Salaried associates are automatically enrolled in a Company-provided disability plan, and will be defaulted into the Tax on Plan Cost option for LTD.

 Part-time Hourly Associates 
Medical, Dental, Vision, Critical Illness Protection and Medical Payment Plan
To have coverage this year, enroll before your 29th day of employment. You will automatically be enrolled in associate only Medical coverage, under the Kaiser Permanente Point of Service option. If you do not want coverage you must complete the legally required HC-5 form and opt out of coverage during your enrollment. Note: Dependents of part-time associates are not eligible for coverage under the Medical plan.
Life Insurance To have coverage this year enroll before your 29th day of employment.
All Other Plans For all other benefits, you can enroll throughout the year once eligible.
