If you enroll in a Home Depot Medical plan* for coverage that begins on January 1, 2016, you will need to complete the Hidden Health Risk Screening by February 5, 2016 to continue receiving your Healthy Living Credit. If you elect the Credit for your spouse or domestic partner, he/she will receive a home mailer with instructions on how to complete the Screening step and the same deadlines will apply.
This valuable Screening is free for those enrolled in a Home Depot Medical plan and is available to other associates at a discounted rate of $45. For 3 ways to complete the Screening, see below.
*The $25 biweekly Healthy Living Credit is available to full-time associates (and their spouses/domestic partners) who are enrolled in a Home Depot Medical plan aside from the Bronze High Deductible Health Plan if they complete the Credit steps as outlined here.
Note: If your physician believes that participating in the Screening is medically inappropriate in your particular situation, you can complete a Physician Waiver Form (see below). Participation in the Healthy Living Credit steps is completely voluntary. Choosing not to participate will only result in individuals not receiving the credit.
Note: Your individual health information is confidential and will be used and disclosed to program administrators only as necessary to administer this program and The Home Depot Medical Plan, as set forth in the Health Plan's Notice of Privacy Practices located in the Benefits Summary. The Healthy Living Credit ID has changed for 2016 to an assigned number rather than a combination of associate ID and birthdate. For those who plan to attend an on-site even at their home location, the ID will be printed on the pre-printed requisition forms for those enrolled in a full-time Home Depot medical plan. Individuals who need their Healthy Living Credit ID (for example to make a PSC appointment or Screen at their doctor's office) can obtain it by logging on at livetheorangelife.com and choosing the Healthy Living Credit tile or by calling Quest Diagnostics at 1-800-354-1703 (press 1) and providing identifying information.
On-site events will take place between January 4 and 29, 2016. On the day of the event, remember to fast for at least 9 hours, continue taking medication as directed by your doctor and drink plenty of water.

Full-time associates and spouses/domestic partners enrolled in a Home Depot Medical plan can participate in the Screening for free at any Quest Diagnostics® Patient Service Center (PSC) that offers biometrics. To find a PSC near you, use the Quest Patient Service Center Lookup Tool. You will then need to schedule an appointment to complete your screening at a PSC (instructions below).

Associates and spouses/domestic partners can have their doctor perform the screening beginning Dec. 1, 2015. There is a very specific process that must be followed. Use the kit below for specific instructions for your doctor to perform the screening.
Note: If you have already had your annual preventive care visit, your doctor's office may charge an office visit copay. In addition, your doctor may apply a fee for completing the form. You are responsible for paying any such copays and/or fees.
If your physician believes that participating in the Screening is medically inappropriate in your particular situation, you can complete a Physician Waiver form. A physician's signature is required and the form should be submitted using the Fax Cover Sheet in the link below.
The communication method preference you provide in the OrangeLife Questionnaire will determine how your Screening results are delivered. If you request email or text message preference, you will receive an email or text message letting you know when your results are ready and directing you to log on to livetheorangelife.com to view them. If you select paper/mail preference, your screening results report will arrive at your home address on file within 3 weeks after your screening. Your report will display results from the current and prior year (if applicable) to help you see your year-over-year progress.
