Important Enrollment Deadlines

If you don't enroll in the following plans by your deadline (within 30 days after your date of hire), you will not be able to enroll until the next Annual Enrollment period unless you have a qualified status change. To view your deadline, create an account and log on at livetheorangelife.com. If you enroll by your deadline, coverage beings on your date of hire.

 Full-time Hourly and Salaried
Medical and Rx, Dental,
Vision, Spending Accounts and Critical Illness Protection Plan
To have coverage this year, enroll by your deadline.
Disability Coverage* You are automatically enrolled in this plan. If you do not want coverage, you must opt out by your deadline.
Life Insurance You have a one-time opportunity to enroll in Basic and Supplemental Life Insurance (coverage of up to three times your pay not to exceed $500,000) without having to provide an Evidence of Insurability form if you enroll by your deadline.
FutureBuilder 401(k) You may enroll at any time on or following your date of hire. Your participation will be effective once you enroll, your paycheck deductions begin, and FutureBuilder receives your first contribution. Paycheck deductions begin as soon as administratively practical following receipt of your enrollment information. Please refer to the FutureBuilder section of the Benefits Summary for matching contribution eligibility information.
All Other Plans For all other benefits, you can enroll throughout the year once eligible.

* Salaried associates are automatically enrolled in a Company-provided disability plan and will be defaulted into the Tax on Plan Cost option for LTD.

Change to Benefits Eligibility Waiting Period for 2018

In the Eligibility and Enrollment and FutureBuilder Chapters of the 2018 Benefits Summary, there is reference to a 90-day waiting period for benefits eligibility for certain benefit plans. Effective June 1, 2018, this is no longer a 90-day waiting period for the benefits specified. Instead, benefits are effective as of your date of hire, subject to any other eligibility and administrative requirements.
